Determining whether Neuropsychological or Psychological Assessment or Treatment is appropriate.
Determining whether you have insurance coverage. Providing the name & card number of your insurance & your DOB will speed up the process.
Dr. Klein will need to speak with you by phone at some point. Please provide multiple numbers if possible. Indicate if it is OK to leave a message, best time to call & how late in the evening to call.
